NATIONAL DIABETES PREVENTION PROGRAM
CLASSES BEGIN IN JANUARY 2020
DAY/TIME: Mondays from 7:00 - 8:00 P.M.
The Centers for Disease Control and Prevention (CDC) developed this wonderful Lifestyle Change Program. It is a year-long program designed for people with prediabetes. This program was developed following a comprehensive review of over 25 years of scientific research on strategies that have been shown to be effective in preventing type 2 diabetes. As a certified Lifestyle Coach, I’m trained to lead groups of participants through this year-long program. Using an evidence-based curriculum, I will provide support and guidance to participants and help them develop and maintain the skills needed to adopt a healthy lifestyle.
The goal of this program is for participants to achieve moderate weight loss by eating well and being active. The goal is for participants to:
Lose at least 5 to 7 percent of their starting weight
Get at least 150 minutes of physical activity each week, at a moderate pace (approximately 20-25 minutes a day)
Decrease symptoms associated with prediabetes
Learn how to control blood glucose with diet and exercise
Improve total cholesterol, lower triglycerides and LDL levels
Improve mental function and memory
Learn strategies to live a healthy lifestyle
WHAT IS prediabetes?
Diabetes is a chronic disease that causes blood sugar levels to rise higher than normal. Diabetes affects more than 29 million people. A condition called prediabetes occurs when blood sugar levels are higher than normal but not high enough for a type 2 diabetes diagnosis. More than 84 million Americans have prediabetes. Without lifestyle changes, people with prediabetes are very likely to progress to type 2 diabetes. The good news is that prediabetes can be reversed.

Each participant will receive materials to help support program goals. The group will meet as follows:
Once a week for four months (sessions 1 to 16)
Every other week for two months (sessions 17 to 20)
Once a month for six months (sessions 21 to 26)
